Faith Formation Registration Form

Welcome to St. Mary Faith Formation! Our K-5th grade session meets on Wednesdays from 5:30-6:45PM and Youth Ministry (6th-12th) meets Wednesdays from 7:30-8:30PM. Catechesis of the Good Shepherd or CGS (ages 3-5) meets Wednesday mornings starting at 9:00AM. Use this online form to register your child.
